• ACTIVE DUTY SERVICE MEMBERS and VETERANS

1. Currently Serving or Honorably discharged with at least 1 year active duty
2. Experiencing financial hardship due to being wounded, injured or illness
3. Must provide copy of DD-214 certificate and/or Military ID
4. A brief description of your emergency needs and reason for the funds being requested

• FIRST RESPONDERS

1. Currently employed or retired as a full time or volunteer First Responder
2. Provide proof of employment or volunteer status on a department letterhead from employer and signed by current department Chief or immediate officer in charge
3. Copy of original certificate or identification card stating title and rank
4. A brief description of your emergency needs and reason for the funds being requested
